Required forms for ALL campers: Now only ONE medical form must be completed in full prior to any camper attending their first day of camp:
Physician Health Form: This form must be downloaded here, printed, completed by your health care professional and mailed back to BB&N. Please note: that some Physicians prefer to use their own Health forms for their patients rather than use a school or camp specific form. This is fine as long as the form is complete and up to date. If you are a BB&N Student, you must still provide the Summer Camp with a copy of this form. We will not be referencing BBN school records for this information. Please note: as of 2010 new requirements are in place with regard to immunizations for children including measles vaccines. You will be contacted by our camp nurse should additional immunizations be required of your child to comply with these new standards. Click here to download the BB&N "Physician Health Form"

Medication? If your child requires ANY* medication during their stay at camp, please download the following document, complete and return to the Camp Nurse before your child's attendance at camp:
* Please note: medication refers to all prescription medication as well as all over-the-counter medication, including all cough and cold remedies.
Sunscreen: All campers should come to camp with sunscreen and bug spray already applied at home. We will not provide a second application of sunscreen or bug spray unless you have supplied us with the lotion/sprays and appropriate directions. All lotions/sprays from home should be marked with the camper's first and last name. Please note: bug repellent containing DEET in concentrations of 30% or less is recommended for protection from insects.
Inhaler? If your child will require the use of an Albuterol Inhaler at camp, please download the following document, complete and return to the Camp Nurse two weeks prior to the start of camp: Medication Administration Care Plan - Albuterol
Epi Pen? If your child will require the use of an epi pen at camp please download the appropriate Medication Care Plan (regular or junior) and the appropriate Allergy Action Plan (regular or junior) as provided below. Debbi Dzierzak: Debbi Dzierzak is excited to be returning this summer as the Pre-Camp Nurse (Pre-Camp Week 1 and Pre-Camp Week 2). Debbi is the Director of Health Services for BB&N based out of the Middle School where she will also be the MS Campus Nurse during the school year. Debbi has 10 years experience as the Nurse Manager at Cape Cod Sea Camps and loves working with the BB&N Summer Camp community. In her spare time she enjoys spending time with her family, reading, golfing and walking the beach. Judith Koeckhoven (BSN, RN): Judith will be working the majority of Summer Camp (Camp Weeks from Week 1 to Week 6 and Post Camp Week 2) Judith is also known as "Nurse K" and is our Lower School's Campus Nurse at BB&N. Judith has worked with our Summer Camp in the past and is looking forward to meeting all of her summer campers and staff members this year. In addition to Judith's extensive nursing career, she is a known Guest Artist and Master Ballet Instructor in the greater Boston area.
Julie Lindstrom, MS,APRN,PNP: Julie will be working Camp Week 7 and 8 as well as Post Camp Weeks this summer. She is a current BB&N Upper School Nurse after a long tenure as our Middle School Nurse. Julie has over 25 years experience as a camp nurse and is very excited to be working at Summer @ BB&N this year to wrap up our season. Julie is celebrating her 20th summer as a nurse at Cape Cod Sea Camps - (10 of them!!) as Nurse Practitioner/Nurse Manager. She enjoys going to the beach, spending time with family & friends, reading and baking.
